Cary J. Kohlenberg, M.D.

- About Cary
Dr. Kohlenberg, a graduate of the University of Wisconsin School of Medicine and Public Health, completed his internship and psychiatric residency training through UW affiliated programs in Milwaukee. He is dual Board Certified in General and Geriatric psychiatry and is a Distinguished Fellow of the American Psychiatric Association. He serves as Assistant Clinical Professor at the Medical College of Wisconsin in both the Departments of Family Medicine and Psychiatry. He is the Medical Director of Independent Psychiatric Consultants and IPC Research. He is frequently asked to lecture on Alzheimer’s disease and other topics, and in 2011 was named the “Practitioner of the Year” by the Alzheimer’s Association of Southeastern WI. Dr. Kohlenberg has been the Principal Investigator in over 100 clinical trials of promising medications, many for the treatment of Alzheimer’s disease.

About Daniel
As the owner and administrator of the clinic, I combine many roles. I serve as Clinical Supervisor, along with providing counseling for both substance use disorders and mental health issues, the only counselor in Dodge County dually licensed to treat both issues. I am certified in trauma counseling, anger management, treatment of anxiety disorders, and telehealth. I also have special training in the treatment of depression and can make use of spiritual and religious approaches when appropriate and desired. I also handle the budgeting, payroll, and bill-paying duties, including my least-favorite job, balancing the checkbook.
I worked for the Department of Corrections for a number of years as a Drug counselor and Social Worker, and then as the Clinical Supervisor of a Methadone Clinic. I'm big on the importance of meeting clients where they are, while recognizing and affirming their desire to become more than what they are.

About Robert
Robert is a psychiatric mental health and adult-gerontology nurse practitioner. He received his Bachelor's degree in nursing in 2008 at Marian University in Fond du Lac and worked as a registered nurse in intensive care and correctional care before going on to pursue his Master's degree at Marian. After receiving certification as an adult-gerontology nurse practitioner he worked in primary care for several years before returning to study mental health at the University of Wisconsin Madison where he received his psychiatric certification in 2018. He is now a clinician in mental health care with experience in treating mood and trauma related disorders, anxiety, schizophrenia, brain injury, developmental and intellectual disabilities, and dementia care.

About Melissa
As a Licensed Professional Counselor, I obtained my undergraduate degree in Psychology from Upper Iowa University, and my Master's degree in Counseling from Lakeland University. I am a Certified Youth Mental Health Counselor as well.
Life is messy, hard, and complicated for all of us for so many different reasons. My goal as a therapist is to help each of my clients navigate their specific, messy, hard, complicated life in the best way they can. Asking for help is scary, but I think it's incredibly brave. I strive to tailor therapy to be what my clients each need as individuals by offering a safe space, free from judgement, to help people work through whatever it is they are struggling with. I have worked with clients of all ages and struggles, though I find myself recently working mainly with children, teens, and young adults struggling with anxiety disorders, OCD, ADHD, and the uncertainty of the current world we live in. Life is hard, but you don't have to struggle alone.

I am trained to work with all mental health issues, and I am certified in anxiety management, Dialectical Behaviour Therapy, and telehealth issues.
I have experience with, schizophrenia, divorce, relationship problems, communication breakdown, cultural and gender bias, developmental issues, existential crises, low self-esteem, skill and coping strategy management, reality testing, mixed family integration, substance abuse, sexual and intimacy problems, and PTSD.
I treat a broad range of clientele, including men, couples, families, young people, and individuals from diverse multicultural backgrounds and nations of origin. My training has been focussed on Person-Centered Therapy, although I am familiar with other techniques including Cognitive Behaviour Therapy and Solution Focused Therapy.

Maggie holds a master’s degree from Concordia University Wisconsin in Counseling in Counseling Education Science and a Bachelor’s degree in Business Administration/ Marketing from Hofstra University on the East Coast. She is a licensed Clinical Substance Abuse Counselor and has been working in the field of addiction for almost a decade. Maggie sees’ a wide variety of clientele and is dedicated in reducing the stigma around addiction. She uses a variety of treatment modalities such as motivational interviewing, cognitive behavioral therapy and dialectical behavioral therapy. Maggie is also a certified anger management professional and has attended workshops dealing with trauma and memory.

Danielle is a professional counselor in training. She obtained her undergraduate and MBA from the University of Phoenix in Healthcare Administration. Danielle then continued to earn a Master of Arts in Counseling from Lakeland University. She has worked in the behavioral health field for 8 years.
Danielle has experience working with anxiety, depression, eating disorders, PTSD, grief, loss, ADHD, bi-polar, schizophrenia, obsessive-compulsive disorder, oppositional defiant disorder, panic/anxiety attacks, LGBTQ community, couples, and trichotillomania disorder. She is also certified in Telehealth Counseling.
She enjoys traveling, camping, outdoor activities and spending time with her family, friends and pets.

Julee graduated with her Master of Science degree from Marquette University in Clinical Mental Health Counseling.
She enjoys working with children, teens, adults, families, and couples. She has experience working with a vast range of mental health diagnoses including anxiety, depression, self-harm/suicidal ideation, trauma, ADHD, ASD-related challenges, abuse, and grief.
Julee works from an eclectic holistic approach that is tailored to the individual, using a variety of methods. Some therapeutic techniques that may be used in therapy are cognitive behavioral therapy, psychodynamic therapy, family therapy, client-centered therapy, and interpersonal therapy.
Julee believes in empowering her clients to reach their goals by using their own expertise and insight and developing coping mechanisms to achieve lasting positive changes. She likes to use humor and be authentic and genuine in the therapy process, which Clients have said they appreciate.

I’m Lisa Derr, a therapist in training and lifelong learner. Being born and raised in Dodge County Wisconsin has given me perspective about the needs of people here. From my early days as a family law mediator and family attorney until pursuing my master’s degree in clinical mental health, I’ve been privileged to work with many individuals and couples in the process of divorce.
As a therapist in training, I now work with married couples to improve their relationship as well as divorcing couples to adapt to their new reality while co-parenting. I also work with individual adults and families, helping them navigate life’s challenges and transitions. This includes families where a child is resisting placement with a parent.
In addition to Cognitive Behavioral Therapy, I have had training in Emotionally Focused Couples Therapy and in Play Therapy for children under 7 years old. I work with a passion for excellence, cultural sensitivity, and an honest desire to help you rediscover hope. Together, we’ll work to create meaningful change.

After his highly decorated 30 year military career, Terry has joined our team at Affiliated Counseling Center, INC as the Office Manager. He is dedicated and determined to provide the highest level of administrative support to our clients and the entire staff. As your first contact with Affiliated Counseling, Terry will go through the intake process with you and then sends out the registration and financial forms that need to be filled out and returned so he can then help you set up your first visit with a clinician. He also assists the clinicians in scheduling/rescheduling client appointments and processing paperwork requests from employers, lawyers and other outside organizations.

About Samantha
After graduating with a degree in Medical Administration, Samantha joined our team at Affiliated Counseling Center, INC. Utilizing her abovementioned education as well as prior office experience, Samantha is responsible for handling all your billing needs to include verification of benifits with insurance companies.
Samantha will also assist the Office Manager with customer service tasks such as scheduleing and rescheduling appointments for clients when they call in, completing initial phone intakes for protential clients and assisting clinicians as needed.
